This agreement authorizes Citizens Bank to deliver your bank statement or bank notice for your account(s) to you, by electronic delivery. An electronic bank statement is called an "E-Statement" and an electronic bank notice is called an "E-Notice". Upon agreeing to this disclosure and successful testing of the electronic delivery method, you will then receive periodic electronic notifications that your statement and/or notice is available to you.
In order for your E-Statement and/or E-Notice to be delivered, the electronic delivery service will need to be tested and validated before electronic delivery can be initiated. During the initial sign up for E-Statement and/or E-Notice, you will need to:
- Select the accounts and documents to enroll
- Verify the email address used to send your notification
- Provide a security phrase, which will show on all automated emails related to E-Statements and E-Notices
- Enter a passcode, which tests your ability to view the PDF format used for document retrieval
- Agree to the “Citizens Bank Electronic Statement and Notice Disclosure”
- Validate the confirmation email you receive once the setup is complete.
Once you have completed the initial enrollment of E-Statements and/or E-Notices, any additional accounts or available notices not previously enrolled, may be set up per your request, by bank personnel. An email will be sent to you with a complete list of all the accounts/notices that have been set up. It is your responsibility to review the email to verify the accounts that have been set up for E-Statements and/or E-Notices.
Although you are electing to receive your bank statement and/or notice electronically, you have the right to have your bank statement and/or notice provided or made available on paper or in non-electronic form. You may submit this request in person, by phone to any Personal Banker at any one of our locations, by email at onlinesupport@citizenbank.com or secure email through your Online Banking account. You will be subject to any fees as indicated by the “Schedule of Fees | Personal Accounts" or "Schedule of Fees | Business Accounts".
You have the right to withdraw your consent to receive your bank statement and/or notice electronically at any time by un-enrolling from the service through your Citizens Bank Online Banking account. You may also contact a Bank employee and request to have your enrollment withdrawn. The E-Statement and/or E-Notice service will be discontinued within 10 working days and you will receive your statement and/or notice in paper form via the United States Postal Service. You will be subject to any fees as indicated by the “Schedule of Fees | Personal Accounts" or "Schedule of Fees | Business Accounts". Any benefits, privileges, or waived fees that may have been provided due to having your statement and/or notice delivered electronically, will no longer be applicable.
You must be a Citizens Bank customer in order to access your electronic bank statement and/or notice. Your computer must meet the following hardware and software requirements for access to and retention of electronic bank statements and notices. While an older operating system and/or browser may still work with viewing your E-Statement/E-Notice, if it were to stop working, the Bank will not be able to help you in finding a solution until you have upgraded your operating system and/or browser to one that is supported.

Native Mobile Apps: only the Citizens Bank-WI Mobile Banking App for iPhone® or the Citizens Bank-WI Mobile Banking App for Android™, available on Google Play™, are supported.
You are responsible for providing us with updated information that we need to contact you electronically, including any change in your e-mail address, etc. We will send you a notice electronically indicating that your statement and/or notice is available. The notice will allow you to log in and retrieve your statement and/or notice electronically.
If we change these requirements, you will be notified at least 30 days before the change goes into effect. If you decide to withdraw your consent to receive your bank statements and/or notices electronically, you will be subject to any fees as indicated by the “Schedule of Fees | Personal Accounts" or "Schedule of Fees | Business Accounts". Any benefits, privileges, or waived fees that may have been provided due to having your statement and/or notice delivered electronically, will no longer be applicable.
We will use commercially reasonable measures, consistent with industry standards, to maintain a reasonable level of security over the information contained in the electronically delivered account statement and/or notice. One of the main security features guarding the use of online banking is the unique combination of your Online Banking ID and Password (PW). It is important that you protect the security of your Online Banking ID and PW by keeping them secret. If you give your Online Banking ID or PW to another party, you will be responsible for any changes or charges to your account(s). By entering into this Agreement and using our Online Banking, you agree to protect the security of your Online Banking ID, PW and any other numbers, codes, marks, signs, keys or other means of identification of you as a Citizens Bank Online Banking customer which may be revised or established from time to time.
By agreeing to the terms and conditions of this disclosure and clicking the button to complete enrollment, you are authorizing Citizens Bank to provide bank statements and/or notices for your chosen accounts by electronic means. You understand and agree that by requesting electronic delivery, you will NOT receive statements and/or notices in paper form that are delivered by the United States Postal Service unless you specifically discontinue the E-Statement and/or E-Notice services as described in this disclosure. If your previous paper statement included checks or imaged check copies, you will NOT see those checks or imaged check copies in your E-Statement. You agree that electronic delivery will satisfy Citizens Bank's requirement to provide you a periodic statement and/or notice on your account. Your consent shall remain valid until such time as you exercise your right to revoke this consent. Citizens Bank retains the right to discontinue this service at any time. Your E-Statement and/or E-Notice service will be discontinued if the e-mail notice is returned undeliverable for any reason. Should your E-Statement and/or E-Notice service be discontinued, Citizens Bank will resume the delivery of your periodic statement and/or notice in paper format via the United States Postal Service. You will be subject to any fees as indicated by the “Schedule of Fees | Personal Accounts" or "Schedule of Fees | Business Accounts". Any benefits, privileges, or waived fees that may have been provided due to having your statement and/or notice delivered electronically, will no longer be applicable.
